17, 2015</b> at the Fairfield Township Police Department,
Walden Glenn and Vonnie Vale Ct in Hamilton, Ohio.</font></div>

photocopy of their current license (if any), original an one
photocopy of any certificates of successful completion of
examination (CSCE) they may hold, positive (photo)
identification, and social security number (SSN) or licence
